This is a guide, not a medical assessment. A prescriber decides whether a medicine is right for you, and they must check your weight properly before prescribing.

Who can get weight loss injections in the UK?

Mounjaro (tirzepatide) and Wegovy (semaglutide) are licensed by the MHRA for weight management in adults, alongside a reduced-calorie diet and more physical activity. The licence covers people with:

BMILicensed?

27 to 29.9Only with at least one weight-related condition
Under 27No

Weight-related conditions include high blood pressure, abnormal blood fats, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes. NHS criteria are narrower than the licence, and they differ across the UK.

Eligibility questions

What BMI do you need for weight loss injections?

Mounjaro and Wegovy are licensed in the UK for adults with a BMI of 30 or more, or a BMI of 27 to 29.9 with at least one weight-related health condition, such as high blood pressure or type 2 diabetes. A prescriber makes the final decision and must check your weight properly.

Who can get weight loss injections on the NHS in Northern Ireland?

The first phase of the Department of Health’s Regional Obesity Management Service is for adults with a BMI over 45 and at least one of: cardiovascular disease, high blood pressure needing medication, abnormal blood fats, sleep-study-confirmed obstructive sleep apnoea, or type 2 diabetes. Rollout was expected from autumn 2026.

Do online pharmacies check your weight?

They should. Guidance from the General Pharmaceutical Council, published in February 2025, says online prescribers must independently verify your weight, height or BMI, for example by video consultation or by checking your records. A questionnaire on its own is not enough.

Is BMI accurate for everyone?

BMI is a rough guide. It does not tell muscle from fat, and health risks start at a lower BMI for people from South Asian, Chinese, other Asian, Middle Eastern, Black African and African-Caribbean backgrounds. NICE guidance uses lower thresholds for these groups, so mention your background to your prescriber.
